About the Role
Aberdeenshire Council is seeking a highly skilled and dedicated Care Manager to join our team. As a Care Manager, you will play a vital role in delivering high-quality care services to older adults and individuals with physical disabilities, including those with acquired brain injury.
Key Responsibilities
- Undertake comprehensive assessments within budgetary constraints to ensure individuals meet service eligibility criteria.
- Develop and implement personalized care plans to meet the unique needs of service users.
- Collaborate with multidisciplinary teams, including social workers, occupational therapists, and nurses, to ensure seamless care delivery.
- Monitor and evaluate care services to ensure quality and effectiveness.
- Contribute to the development and implementation of care policies and procedures.
Requirements
- Hold a recognized qualification in social work, occupational therapy, or nursing.
- Have experience working with older adults and individuals with physical disabilities.
- Be registered with a relevant professional body.
- Hold a current valid driving license.
