Reablement is an integral part of the broader Support @ Home initiative, designed to facilitate recovery from health challenges and to mitigate the risk of health decline whenever feasible. The service provides a tailored rehabilitation period aimed at empowering service users to maintain, regain, or relearn essential daily living skills and capabilities. Reablement programs are customized to align with the objectives established with service users, focusing on helping them reach their full potential while minimizing or delaying the need for long-term community services.

Key Responsibilities

  • Oversee the implementation and enhancement of the therapy aspect of the Intermediate Care and Rehabilitation Services for individuals referred to the reablement team operating within the local authority.
  • Ensure effective treatment and management of service users requiring rehabilitation in the community, aiming to maximize their independence and functional capabilities within their home and local surroundings, thereby preventing unnecessary hospital admissions and facilitating timely discharges.
  • Provide clinical leadership and guidance to a team of Occupational Therapists, Physiotherapists, and Therapy Technicians, while supporting local authority staff to improve service delivery and clinical interventions for community patients.

The ability to communicate in Welsh is preferred for this position; applications from both Welsh and English speakers are welcomed.

Qualifications and Knowledge

Essential

  • Degree/Diploma in Occupational Therapy or Physiotherapy.
  • Demonstrated ability to operate at a Master's Level.
  • Specialized knowledge of professional assessments and interventions pertinent to community rehabilitation.
  • Understanding of current health and social care policy agendas and legislative frameworks.
  • In-depth knowledge of clinical governance and risk management principles.
  • Thorough understanding of collaborative working principles.
  • Evidence of critical thinking and analytical skills.
  • Expertise in specialized rehabilitation methodologies.
  • Commitment to Continuous Professional Development (CPD).

Desirable

  • Management qualifications such as ILM or clinical leadership.
  • Familiarity with standardized and non-standardized assessments applicable to the client group.
  • Interest in quality assurance, audit, and service evaluation.
  • Postgraduate training in Rehabilitation.
  • Comprehensive understanding of professional ethics, standards, and their practical application.

Experience

  • Extensive postgraduate clinical experience in a relevant specialty.
  • Experience in multi-disciplinary and multi-agency collaboration.
  • Significant experience in clinical and team leadership roles.
