The Springfield Park PCN in City and Hackney is seeking a Clinical Pharmacist to develop and support medicines optimisation work within the GP practices in the PCN, covering a population of approximately 56,000.

Main Responsibilities

The Clinical Pharmacist will be responsible for:

  • Undertaking face-to-face medication reviews for patients with long-term conditions on polypharmacy.
  • Reviewing ongoing need for each medicine, assessing monitoring requirements, and supporting patients to ensure treatment is safe, appropriate, and optimised for maximum clinical benefit.
  • Discussing recommendations and outcomes with relevant members of the PCN team and providing appropriate and timely follow-up.
  • Supporting practices in improving patient access for minor ailments and acute presentations.
  • Promoting the importance of self-care and signposting patients to community pharmacy-led services.
Medicines Information and Governance

The Clinical Pharmacist will be responsible for:

  • Supporting resolution of medicines queries from the practice team, PCN team, community pharmacists, and patients.
  • Providing medicines optimisation updates at clinical meetings to inform and improve prescribing within the practice.
  • Reconciling medicines following discharge from hospitals, intermediate care, and into care homes.
  • Communicating, acting, and supporting implementation of MHRA alerts, product withdrawals, and other local and national guidelines.
  • Undertaking clinical audits in priority areas identified by the Medicines Optimisation Team.
Education and Training

The Clinical Pharmacist will contribute to the provision of education and training on medicines optimisation to healthcare professionals within the PCN.

Requirements

The ideal candidate will have:

  • A minimum of 2 years post-graduate experience in pharmacy.
  • An independent prescribing qualification or be working towards one.
  • An understanding of the role of a clinical pharmacist within primary care.
Person Specification

The successful candidate will have:

  • A minimum of 2 years post-qualification experience.
  • Registration as a pharmacist with the General Pharmaceutical Council.
