About the Role

We are seeking a highly motivated and experienced Social Prescriber to join our team at The Forward Trust. As a Social Prescriber, you will play a vital role in supporting individuals with substance misuse issues and low-level mental health concerns.

Key Responsibilities
  • Develop and implement a framework for non-clinical needs that may be met through social prescribing
  • Provide assertive outreach to individuals who have declined or disengaged from health and wellbeing services
  • Engage with service users and peer mentors to identify groups to address social and emotional needs affecting health
  • Provide brief advice, motivational interviewing, and health coaching while maintaining non-judgmental conversations tailored to individual needs
  • Promote local services and develop a social prescribing directory
  • Work in partnership to reduce health inequality and promote health
  • Deliver health promotion events and proactively engage with other service providers, partners, and service user groups
Requirements
  • Experience of working with service user groups within front-line health services
  • Knowledge of the issues facing those with experience of substance misuse and/or low-level mental health issues
  • Demonstratable experience of carrying out risk assessments, comprehensive assessments, and designing and implementing SMART care plans
  • Strong IT skills
  • Experience of carrying out psychosocial interventions, including using motivational interviewing techniques in 1:1 and group settings
