Job Summary
We are seeking a skilled and dedicated Practice Nurse to join our team at Shotfield Medical Practice. As a Practice Nurse, you will play a vital role in providing high-quality healthcare services to our patients.
Main Duties of the Job
The successful candidate will be responsible for:
- Providing a range of quality health and personal care services to patients, including cervical cytology, chronic disease management, patient education, and wound management.
- Working as an autonomous practitioner within the multi-disciplinary team to provide a full range of Practice Nursing services.
- Assessing, implementing, and evaluating clinical packages of care for patients.
- Providing health education and working with patients and other healthcare professionals to effectively deliver appropriate packages of care.
- Advising patients and other clinical staff on the management of chronic diseases and appropriate therapies and pharmacological intervention.
- Referencing patients independently to other clinical disciplines internally and externally as appropriate for further care.
- Undertaking clinical audit activities to review care provided, make recommendations for improvement in care, and implement agreed changes.
- Assisting with QOF and other target work to ensure maximum performance for the practice.

Requirements
To be successful in this role, you will need:
- A Registered Nurse qualification and registration with the NMC.
- Experience in cervical cytology, chronic disease management, patient education, and wound management.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multi-disciplinary team.
- Up-to-date knowledge of NICE guidance and other national guidance.
- Excellent time management and prioritization skills.
