Position Overview
The Clinical Nurse Specialist for the Referral Coordination Hub (RCH) will oversee the bed management operations within the West Midlands Children’s Provider Collaborative (WMCPC). This role encompasses patient flow management, escalation processes, system interactions, and bed capacity oversight, all aimed at minimizing unnecessary admissions and ensuring service users are placed in high-quality, locally contracted beds while eliminating inappropriate out-of-area placements.
Key Responsibilities
- Act as the primary contact for daily management of patient flow and escalation challenges, including bed management. Collaborate with both internal and external stakeholders to reduce waiting times and optimize the use of contracted resources.
- Lead and address escalations while ensuring that effective processes are established and adhered to regarding patient admissions and discharges.
- Utilize digital systems efficiently and in real-time to capture and report accurate data, ensuring that patient flow systems operate safely and to the highest standards, in alignment with Trust values.
- Employ data analytics to forecast activities and foster a proactive culture aimed at improving patient flow.
- Assist the bed management team and referral hub staff in the development, execution, management, and evaluation of clinical and operational strategies, policies, and escalation protocols that facilitate patient flow and discharge processes.
- Represent the WMCPC leadership team in operational management discussions related to patient flow during relevant calls and meetings.
- Ensure that patient bed utilization aligns with key access targets, including admission flow, discharge case management, and overall discharge efficiency, in collaboration with the referral hub management team and WMCPC case managers.
