Job Title: Care Home GP

Job Summary:

We are seeking a highly skilled and experienced General Practitioner to join our team at Harborough Field Surgery. The successful candidate will be responsible for providing high-quality medical care to patients in a care home setting.

Key Responsibilities:

  1. Undertake a variety of duties including surgery consultations, telephone consultations, and visiting patients at home.
  2. Check and sign repeat prescriptions, test results, and deal with queries in a timely fashion.
  3. Work with electronic document management systems, including EMIS Web, ICE, and Choose and Book.
  4. Assess the healthcare needs of patients with undifferentiated and undiagnosed problems.
  5. Screen patients for disease risk factors and early signs of illness in consultation with patients and in line with current practice disease management protocols.
  6. Develop care plans for health and provide counselling and health education as needed.
  7. Admit or discharge patients to and from the caseload and refer to other care providers as appropriate.
  8. Record clear and contemporaneous consultation notes to agreed standards.
  9. Collect data for audit purposes and quality purposes.
  10. Prescribe in accordance with the practice prescribing formulary and generically whenever clinically appropriate.

Requirements:

The ideal candidate will have excellent clinical skills, strong communication skills, and the ability to work independently in a fast-paced environment. They will also have experience working in a care home setting and be familiar with electronic document management systems.
