We are seeking a skilled and dedicated individual to join our team as a District Nurse/ Community Nursing Sister/ Charge Nurse. As a key member of our primary care team, you will play a vital role in delivering high-quality patient care and supporting the health and wellbeing of our community.
Key Responsibilities:
As a District Nurse/ Community Nursing Sister/ Charge Nurse, you will be responsible for:
- Providing expert nursing care to patients in their homes and community settings
- Working closely with other healthcare professionals to develop and implement care plans
- Conducting risk assessments and providing psycho-educational skills to enhance maintenance of wellbeing
- Delivering the service as a standalone intervention and providing psycho-educational skills to enhance maintenance of wellbeing using resources provided
- Developing and maintaining strong relationships with patients, families, and carers
Requirements:
To be successful in this role, you will need:
- A relevant qualification for the role equivalent to (but not specifically) QCF Level 3 in Health & Social Care
- Knowledge of the aims, objectives, and ethos of primary care and the PCN
- Knowledge of the issues of supporting people with a range of mental health issues, including risk assessment
- Knowledge of the Mental Health Sector and the Dorset framework
- Good working knowledge of Microsoft Office 365 applications, email, and the internet
- Strong interpersonal and communication skills, able to work with others as a team
