Key Responsibilities:

As a District Nurse/ Community Nursing Sister/ Charge Nurse, you will be responsible for:

  • Providing expert nursing care to patients in their homes and community settings
  • Working closely with other healthcare professionals to develop and implement care plans
  • Conducting risk assessments and providing psycho-educational skills to enhance maintenance of wellbeing
  • Delivering the service as a standalone intervention and providing psycho-educational skills to enhance maintenance of wellbeing using resources provided
  • Developing and maintaining strong relationships with patients, families, and carers

Requirements:

To be successful in this role, you will need:

  • A relevant qualification for the role equivalent to (but not specifically) QCF Level 3 in Health & Social Care
  • Knowledge of the aims, objectives, and ethos of primary care and the PCN
  • Knowledge of the issues of supporting people with a range of mental health issues, including risk assessment
  • Knowledge of the Mental Health Sector and the Dorset framework
  • Good working knowledge of Microsoft Office 365 applications, email, and the internet
  • Strong interpersonal and communication skills, able to work with others as a team
