{"Job Title": "Outreach Family Support Worker", "Job Summary": "We are seeking an experienced Outreach Family Support Worker to join our team at CRA GROUP LIMITED. As a key member of our Early Help team, you will work closely with children, young people, and their families to provide high-quality support and interventions.

Your primary responsibility will be to undertake early help assessments with families, create support plans, and chair team around the family meetings to review progress and identify additional areas of support.

Key Duties/Accountabilities:

* Take whole case responsibility to provide high-quality outreach and home-based Early Help support to children and families using a whole family approach (level 2 and 3).
* Deliver targeted support to children across age ranges (0-25*) and their families in order to prevent the need for statutory services and to ensure families access Universal services.
* Act as case holders for complex cases that step down from children's social care (level 3/2).
* Provide intensive and targeted support to vulnerable children and young people, including bespoke packages and programmes.

Essential Experience Required:

* Experience of direct work with children, young people, and their families is essential.
* Experience of effective collaborative and multi-agency working to support children and families is essential.
* Experience of delivering outreach and home-based support to children and families to address areas of identified need is essential.

Essential Qualification Required:

* NVQ Level 3 in childcare and education or NND Health and Social Care qualification is essential.

Additional Information:

* Working hours: 35 hours per week.
* The role requires an Enhanced DBS check.
