Care Coordinator Specialist

We are seeking a skilled Care Coordinator to join our team at First City Nursing and Care in Swindon. This is a permanent opportunity to work in a supportive environment, providing excellent care to our clients.
Key Responsibilities- Allocate care workers to customer visits, ensuring continuity of care and considering locations and skills required.
- Communicate regularly with care workers, providing guidance and resolving queries in a timely manner.
- Liaise with other healthcare professionals to ensure seamless care delivery.
- Ensure all paperwork is completed and entered onto relevant systems.
- Maintain confidentiality and approach clients with empathy and understanding.
- Experience working in health and social care, preferably in a community-based setting.
- Computer literacy and good office skills.
- Honest, caring, non-judgmental, and kind attitude.
- Organized and able to work calmly under pressure.
- Excellent training and support.
