About This Role

We are seeking a dedicated and compassionate Occupational Therapist to join our dynamic community therapy team. You will play a vital role in supporting patients as they recover and regain independence within their homes.

Your Responsibilities

  • Provide comprehensive assessments and treatment plans for patients recovering from illness or injury.
  • Collaborate closely with a multidisciplinary team, including Physiotherapists, Therapy Assistants, Social Workers, Community Nurses, and GPs.
  • Develop patient-centered goals and support them in achieving their rehabilitation objectives.
  • Contribute to the discharge home to assess service, ensuring smooth transitions for patients leaving hospital.
  • Supervise and mentor junior staff members, fostering a supportive learning environment.
