About the Role

Priory Medical Practice is seeking a dedicated and innovative Pharmaceutical Care Specialist to join our team on a part-time basis. This role offers flexible hours to accommodate the successful candidate. The primary responsibilities include:

  • Engaging with patients to facilitate effective medication use through collaborative decision-making.
  • Executing tasks related to medication optimization.
  • Assisting with medication reviews and reconciliation for new patients in care facilities, ensuring seamless transitions of care by coordinating with local pharmacies.
  • Supporting the clinical team in conducting Structured Medication Reviews.
  • Contributing to the delivery of medicines optimization, management incentive programs, and patient safety evaluations.
  • Implementing national prescribing policies and guidelines within the practice.
  • Collaborating with the multi-disciplinary team to ensure adherence to efficient medicines optimization processes.
  • Establishing effective ordering and return systems to minimize medication waste.
  • Promoting Electronic Repeat Dispensing (eRD) and online prescription requests.
  • Assisting practice reception teams in managing prescription requests.
  • Facilitating the application of national prescribing policies and guidelines across various healthcare settings.
  • Utilizing diverse communication methods while recognizing and addressing potential barriers.
  • Providing essential information to patients, their caregivers, or family members on behalf of the healthcare team.
About the Candidate

The ideal candidate will possess the following qualifications, skills, and experience:

  • Professional registration with the General Pharmaceutical Council (GPhC).
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Demonstrated commitment to continued professional development (CPD).
  • Experience as a qualified, registered pharmacy technician in primary care, community, or hospital settings.
  • Ability to effectively influence and persuade stakeholders regarding various options, innovations, and challenges.
