We are seeking a highly skilled and experienced Senior Occupational Therapist to join our Dementia Team in Shropshire and Telford. As a key member of our team, you will play a vital role in supporting individuals affected by dementia, providing specialist assessments, and developing personalized care plans to maximize their independence and quality of life.
Main Responsibilities- Conduct comprehensive assessments to identify mental health needs and develop tailored care plans in collaboration with service users, carers, and multidisciplinary teams.
- Deliver a range of non-pharmacological interventions to address unmet needs and promote occupational performance and independence.
- Manage a complex caseload, prioritizing workload, and allocating tasks to ensure timely and effective care delivery.
- Collaborate with stakeholders, including families, carers, and local communities, to ensure seamless care coordination and support.
- Stay up-to-date with national, regional, and local drivers underpinning service delivery, including the Mental Health Act 1983 and the Mental Capacity Act 2005.
