About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Specialist to join our team at Avon & Wiltshire Mental Health Partnership NHS Trust. As a key member of our Primary Care Liaison Service, you will play a vital role in providing mental health assessments and interventions to patients and their families in community settings.

Key Responsibilities
  1. Provide comprehensive mental health assessments and risk management strategies to patients and their families in community settings.
  2. Foster strong collaborative relationships with primary care health professionals, providing them with credible clinical expertise and educative support.
  3. Assist primary care and secondary mental health teams in delivering safe and effective care planning processes.
  4. Provide concise and effective liaison and feedback to clinical staff within secondary mental health services and primary care teams.
  5. Contribute evidence-based expertise to multi-disciplinary team care planning processes.
  6. Develop and maintain strong communication with service users, relatives, and carers, primary care and secondary health services.
  7. Promote positive attitudes, mutual understanding, and collaboration between non-mental health staff and mental health services, users, carers, voluntary agencies, primary care, and social services.
  8. Provide mentoring and training to others in relevant practice areas, taking a collaborative approach to practice development and evidence-based care.
  9. Develop own knowledge and practice, and contribute to the development of others, making use of available feedback, supervision, and appraisal to identify areas for development.
  10. Maintain accurate and up-to-date single health and social care records, adhering to professional and organisational standards.
  11. Assist in the development and implementation of systems to ensure a consistent approach to assessment, communication, and follow-up.
  12. Participate in local arrangements to manage unexpected staff absences.
  13. Be able to complete assessments across a largely rural area, with access to a vehicle to facilitate this.
Desirable Qualifications and Experience
  • Relevant post-registration qualifications, such as Thorn, Non-Medical Prescribing, CBT, AMHP, Brief Interventions, etc.
Essential Knowledge and Skills
  • Detailed working knowledge of secondary specialist mental health services, including their organisation and operation, and how they fit into the health and social care community.
  • Demonstrates substantial experience of assessing risk and developing risk management strategies.
