About the Role

We are seeking a skilled and compassionate Community Nurse to join our team at Sirona care & health CIC. As a Community Nurse, you will play a vital role in providing high-quality care to individuals in their own homes, working collaboratively with our Integrated Neighbourhood Teams.

Key Responsibilities
  • Provide holistic and specialist nursing care to service users in their own homes, working within a multi-disciplinary team.
  • Develop and implement individualized care plans, taking into account the unique needs and circumstances of each service user.
  • Work closely with other healthcare professionals, including therapists and support workers, to ensure seamless care delivery.
  • Contribute to the development and implementation of clinical policies and procedures, ensuring best practice and high-quality care.
  • Participate in ongoing professional development, staying up-to-date with the latest clinical guidelines and research.

Requirements

To be successful in this role, you will need:

  • BSc or equivalent in adult nursing and NMC registration.
  • Effective communication skills, with the ability to adapt communication for individuals with communication difficulties.
  • Able to make clinically reasoned judgements when working with a variety of conditions.
  • Ability to work autonomously, with a willingness to seek advice and guidance when needed.
