Care Coordinator Role

We are seeking a highly organized and efficient Care Coordinator to join our team at Universal Care Agency. As a Care Coordinator, you will play a vital role in ensuring our carers provide effective and safe individual care to our service users.

Key Responsibilities:
  1. Liaise with the Registered Manager to ensure all carers are following agreed care plans.
  2. Maintain full and complete knowledge of service users, including their care needs, care plans, assessments, and schedules.
  3. Update care plans, risk assessments, and service user details in a timely manner.
  4. Ensure compliance with CQC regulations and Company policies and procedures.
  5. Attend meetings with the management team, service users, and their families to ensure the service meets their needs.
  6. Coordinate with local community teams, such as GPs, Hospitals, and District Nurses.
  7. Provide care in the absence of carers to the expected standards.
  8. Maintain an organized office environment and be the first point of contact for scheduling service user visits.
  9. Minute staff meetings and keep accurate records of carer's mandatory training.
  10. Share on-call duties to cover absences as required.
Requirements:
  1. Knowledge of care plans and the CQC framework.
  2. At least 1 year's work experience in the Care sector.
  3. Reliable, own car for work purposes.
