About the Role

We are seeking an experienced and dedicated professional to join our Hospital Discharge team as a Social Worker. As a key member of our team, you will play a vital role in supporting individuals to achieve their goals and regain independence.

Key Responsibilities
  • Conduct assessments and develop discharge plans to ensure individuals receive the necessary support to return home safely.
  • Work closely with healthcare professionals, families, and carers to coordinate care and support.
  • Provide emotional support and guidance to individuals and their families during a vulnerable time.
  • Collaborate with the reablement team to ensure a smooth transition from hospital to home.
  • Manage a caseload and prioritize tasks to meet deadlines and deliver high-quality outcomes.
Requirements
  • Recognized social work qualification and registration with Social Work England.
  • Experience working in a hospital discharge or reablement setting.
  • Strong communication and interpersonal skills.
  • Ability to work under pressure and manage competing priorities.
  • Valid driving license and access to a vehicle.
