About the RoleThe successful candidate will work as part of a multidisciplinary team alongside GP's, Nurses, and healthcare assistants, providing high-quality patient care and contributing to the practice's goals.
Key Responsibilities- Deliver exceptional patient care, working closely with the practice team to manage chronic diseases, such as diabetes and asthma, and provide vaccinations and immunisations.
- Contribute to the practice's quality improvement initiatives, staying up-to-date with clinical updates, immunisations, and vaccinations.
- Work collaboratively with the practice team to provide family planning services and cervical cytology.
- Knowledge and experience of chronic disease management, including diabetes, asthma, and COPD.
- Ability to work as part of a multidisciplinary team, communicating effectively with colleagues and patients.
- Up-to-date knowledge of QOF and Enhanced Services.
- Full-time position with a competitive salary of £35,000 p/a, negotiable based on experience.
