Community Nurse Case Manager Opportunity

We are seeking an experienced RGN or RSCN Qualified Nurse to join our client as a Community Nurse Case Manager in the North West. This role offers a chance to make a real difference in the lives of individuals with complex care needs.

About the Role

As a Community Nurse Case Manager, you will be responsible for conducting initial assessments in the community, training and assessing workers' competencies, and collaborating with the wider multidisciplinary team. You will also manage a team of non-clinical case managers and hold team meetings.

Package and Benefits

The Community Nurse Case Manager role comes with a comprehensive package, including:

  • Annual salary of £43,000
  • £3,000 car allowance
  • Mileage expenses
  • Full-time hours: 37.5 hours per week, Monday to Friday
Requirements

The ideal candidate will be an RGN or RSCN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management. You will also have experience in tracheostomy and complex care, and be a car driver, flexible to travel around the North West.
