About the Role:
We are seeking an experienced Care Coordinator to join our dedicated team in St Helens, Merseyside. As a skilled Care Coordinator, you will play a key role in the continuous delivery of care to service users.
Key Responsibilities:
- Manage a caseload of care packages and understand the varied nature and fast-pace of the role.
- Apply communication and relationship building skills to effectively manage rota's for care teams.
- Respond efficiently to any day-to-day changes and act as the first point of contact for service users, their families and other healthcare professionals.
- Work well under pressure, using initiative and thinking on your feet.
Requirements:
- Experience in care coordination, preferably in a similar setting.
- Excellent communication and interpersonal skills.
- Ability to work well under pressure and adapt to changing situations.
- Valid driving license and access to a vehicle.
- Commitment to supporting the out of hours on call service.
What We Offer:
- Competitive salary between £29,460 - £35,460 per annum.
- Payment for on-call.
- Bonus scheme up to £2,000 per annum.
- 25 days annual leave, plus Bank Holidays.
