We are seeking a highly skilled and compassionate Support Worker to join our Oaklands and St Kilda Service in Ealing. As a Support Worker, you will play a vital role in supporting young people aged 18-25 who may have experienced mental health-related hospital admissions to gain and develop skills for future independent living.
Key Responsibilities- Manage a caseload of young people, providing individualized support to help them achieve their goals and develop meaningful relationships.
- Work collaboratively with other team members to handle administrative responsibilities and ensure the smooth operation of the service.
- Conduct key work sessions, using innovative and engaging approaches to achieve Support Plan goals.
- Co-produce ongoing assessment and management of risks with young people, adopting a positive risk-taking attitude.
- Provide support with daily living activities, including practical assistance to help young people develop independent living skills.
- Support young people in maintaining a healthy environment, including cleaning their accommodation and promoting good personal hygiene.
- Assist in recording and reporting customer incidents.
- Manage referrals, complete assessments, and maintain relevant paperwork.
- Develop productive relationships with partner organizations to improve service outcomes.
- Adhere to lone working protocols and maintain high levels of awareness regarding the health and safety of yourself and colleagues.
- Involving young people in the design, development, and delivery of the service.
- Encouraging and enabling tenants to pay their rent and avoid personal debt.
- Ensuring Look Ahead Health and Safety policies and local protocols are adhered to at all times.
- Taking on shift leading responsibilities by coordinating and leading shifts in the absence of a manager.
- Up to NVQ Level 2/3 or equivalent, or experience supporting young people or individuals experiencing mental health challenges.
- Experience working in complex mental health environments.
