Key Responsibilities

This newly created role presents an exciting opportunity for a motivated pharmacy technician aiming to enhance their skills while supporting the growth of integrated pharmacy services within frailty care.

The post holder will collaborate closely with the lead pharmacist for frailty and out-of-hospital care to innovate and advance the frailty service, with the goal of minimizing emergency department visits, reducing hospital admissions, and ensuring patients maximize their medication benefits.

The responsibilities include:

  • Providing high-quality medication services to the frailty virtual ward.
  • Actively participating in the daily operations of the virtual ward.
  • Working in partnership with a diverse team to manage all aspects of medication services for patients.

Qualifications and Experience

Essential:

  • Registered with the General Pharmaceutical Council as a pharmacy technician.

  • Experience in frailty and elderly care services, particularly in patient counseling and medication reconciliation.

Desirable:

  • Proven ability to utilize clinical audit and quality improvement methodologies to enhance practice.
  • Understanding of integrated pharmacy services and capability to identify and plan service improvements.
