Harrogate and District NHS Foundation Trust is seeking a highly skilled Health Visitor to join our County Durham 0-25 Team based in Brandon Family Centre in Durham. As a Health Visitor, you will play a vital role in promoting positive holistic health care for an identified population and community.

Main Duties of the Job- Plan, organise and deliver programmes of care in conjunction with the family and the multi-disciplinary team.
- Undertake continuous on-going assessment of children, young people and families.
- Provide an evidenced-based service to children, people, families and communities in accordance with National and Local Policy initiatives.
