Position Overview

This is a pivotal opportunity to become part of a dynamic team as we seek to enhance our service offerings.

The Nottingham CityCare Urgent Treatment Centre is a clinician-led facility dedicated to the assessment and management of individuals with urgent yet non-life-threatening health concerns.

This role is ideal for a passionate, proactive, and adaptable Registered Health Care Professional with proven experience as an independent prescriber (V300) in an urgent care setting.

We also welcome applications from Emergency Care/Nurse Practitioners with V300 prescribing qualifications who are eager to broaden their experience with patients presenting with various health issues while also addressing minor injuries.

The successful candidate will engage in Continuous Professional Development (CPD), which includes a combination of self-directed learning and mentorship, supported by both internal and external training relevant to their role. Flexibility is essential, as the position requires working throughout the year with shifts scheduled from early morning to late evening. Both full-time and part-time opportunities are available.

The Urgent Treatment Centre serves as a training hub where employed practitioners will contribute to the professional growth of various healthcare disciplines, including student nurses, pharmacists, GP SpRs, medical students, and paramedics.

The facility has recently undergone modernization and is equipped with on-site X-ray capabilities.

Main Responsibilities

Informal visits to the Urgent Treatment Centre are encouraged.

As an autonomous practitioner and prescriber, you will evaluate patients presenting with urgent but non-critical conditions, devise care plans, administer treatments on-site, and provide discharge support to junior staff as necessary. When on-site treatment is not suitable, you will refer patients to other healthcare professionals or external agencies. You will also take on a leadership role for a small clinical team.

Applications will be considered for both full-time and part-time positions.

Key Responsibilities

Role Purpose

The post holder will manage an urgent care caseload in a clinician-led environment, addressing a variety of presenting undifferentiated conditions. You will collaborate with a multidisciplinary team of clinicians, including those with acute and primary care first contact experience, nurses, paramedics, healthcare support workers, and emergency care practitioners, to collectively assess urgent care patient presentations.

Your responsibilities will include:

  • Conducting clinical assessments and delivering treatments to service users with undifferentiated healthcare needs to support the primary care team and prevent unnecessary hospital admissions.
  • Utilizing advanced clinical skills and competencies in health assessment, medication management, leadership, and case management.
  • Maximizing patient health and minimizing risks that contribute to ill health, thereby reducing unnecessary admissions to acute services.
  • Leading the development of specific clinical areas such as pediatrics, sexual health, and safeguarding, while being an autonomous practitioner open to the flexibility required for the role.
  • Participating in training opportunities for yourself and others within the Urgent Treatment Centre.
  • Providing mentorship and support to staff within the Urgent Treatment Centre and the broader healthcare community.
  • Taking operational charge of a shift on a planned rota basis in the absence of the Urgent Treatment Centre Coordinator.
  • Providing line management to clinical practitioners and non-registered staff.

Dimensions

You will work with the CityCare multidisciplinary team to provide primary healthcare to residents and visitors in the region. You may also represent Nottingham CityCare Partnership at local, regional, and national levels as required.

Clinical Responsibilities

Your clinical responsibilities will include:

  • Comprehensively assessing, reviewing, and evaluating the needs of service users with undifferentiated healthcare needs to enhance their physical, psychological, and mental well-being, promoting self-care and reducing the need for hospitalization.
  • Accurately prioritizing patients within the service, conducting clinical assessments, and providing necessary treatments, including the use of Non-Medical Prescribing and/or Patient Group Directions in accordance with local and national policies.
  • Carrying out diagnostic tests only when clinically indicated, following organizational, local, and national guidelines appropriate to the Urgent Treatment Centre setting.
  • Developing care plans in collaboration with service users, applying clinical knowledge, analyzing symptoms and data, identifying risk factors, recognizing early signs of acute illness, and ensuring patient and carer involvement in the care plan.
  • Utilizing effective communication to facilitate onward referrals to specialists as appropriate, ensuring a coordinated primary healthcare response to service users' needs.
  • Working autonomously without direct supervision while recognizing the limitations of the practitioner role.
  • Performing enhanced clinical skills as required, which may include suturing and fracture management.
  • Maintaining accurate and immediate records of all interventions, fully utilizing the software systems in place.
  • Encouraging clients to engage in self-care through effective communication and providing written information as necessary.
  • Advising clients on the appropriate use of primary, secondary, and emergency services.
  • Upholding patient confidentiality in accordance with Urgent Treatment Centre and CityCare clinical guidelines.
