Job Title: Associate Hospice Nurse Specialist

Trinity Hospice & Palliative Care Services is seeking an experienced Associate Hospice Nurse Specialist to join our dynamic team. As a key member of our community team, you will be responsible for delivering high-quality care to patients with life-limiting illnesses.

About the Role

This is an exciting opportunity for a skilled nurse to develop advanced clinical skills and work collaboratively with our team to support the development of Single Point of Access and Virtual Ward projects. You will also be involved in delivering education to enable acute and community colleagues to support their patients to 'live well'. Additionally, you will work closely with the Palliative Care Networks across the area to support the strategic vision of care needs.

Key Responsibilities
  • Deliver high-quality care to patients with life-limiting illnesses
  • Develop advanced clinical skills and work collaboratively with the team
  • Support the development of Single Point of Access and Virtual Ward projects
  • Deliver education to enable acute and community colleagues to support their patients to 'live well'
  • Work closely with the Palliative Care Networks across the area to support the strategic vision of care needs
