Job Title: Advanced Nurse Practitioner

We are seeking an experienced and skilled Advanced Nurse Practitioner to join our team at Brook Medical Centre.

About the Role

The successful candidate will be responsible for providing high-quality patient care, managing a range of acute and chronic medical conditions, and developing and implementing health promotion and wellbeing programmes.

Key Responsibilities
  1. Develop, implement, and embed health promotion and wellbeing programmes to improve patient outcomes.
  2. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice and care.
  3. Implement and evaluate individualised treatment plans for chronic disease patients, ensuring effective management and prevention of complications.
  4. Identify, manage, and support patients at risk of developing long-term conditions, preventing adverse effects on their health.
  5. Provide advanced, specialist nursing care to patients as required, in accordance with clinical evidence, NICE guidelines, and the NSF.
  6. Provide wound care, including ulcer and Doppler assessments, and implement effective treatment plans.
  7. Deliver specialist clinics, such as ear syringing, contraception, and travel medicine services.
  8. Collect and process pathological specimens, and request pathology services as necessary.
  9. Process and interpret pathology and other test results, ensuring accurate diagnosis and treatment.
  10. Provide chronic disease clinics, delivering patient care and referring patients to secondary or specialist care as required.
  11. Maintain accurate clinical records, adhering to extant legislation and ensuring confidentiality.
  12. Ensure the effective use of SNOMED CT codes in clinical documentation.
  13. Maintain chronic disease registers, ensuring up-to-date records and effective management.
  14. Develop, implement, and embed well woman, well man, and travel clinics, promoting health and wellbeing.
  15. Develop and implement an effective call/recall system, ensuring timely follow-up and patient engagement.
  16. Lead the implementation of an effective immunisation programme, ensuring up-to-date vaccination schedules and patient safety.
  17. Chaperone patients as necessary, ensuring dignity and respect.
  18. Prioritise health issues, intervening appropriately to prevent complications and promote patient safety.
  19. Support the team in dealing with clinical emergencies, ensuring effective response and patient care.
  20. Recognise, assess, and refer patients presenting with mental health needs, ensuring timely intervention and support.
  21. Implement vaccination programmes for adults and children, ensuring up-to-date schedules and patient safety.
  22. Act as an extended and supplementary prescriber, adhering to extant guidance and ensuring patient safety.
  23. Support patients in the use of their prescribed medicines or over-the-counter medications, reviewing annually as required.
  24. Contribute to practice targets, complying with local and regional guidance and ensuring quality patient care.
  25. Liaise with external services and agencies to ensure patient support and coordination.
  26. Delegate clinical responsibilities appropriately, ensuring safe practice and task delegation within scope of practice.
  27. Support the clinical team with safeguarding matters, adhering to local and national policies.
  28. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  29. Deliver opportunistic health promotion, promoting health and wellbeing.
  30. Provide supervision and support to other members of staff, including Healthcare Professionals, Practice Nurses, Nurse Associates, and Nurse Practitioners.
  31. Provide on-the-day care in room 6, in line with practice policy.
  32. Attend clinical, MDT, and practice meetings, ensuring effective communication and collaboration.
  33. Undertake any tasks consistent with the level of the post and scope of the role, ensuring timely and effective delivery.
Additional Responsibilities
  1. Deliver routine ANP appointments, maintaining training standards and adhering to senior partner criteria.
  2. Deliver minor operations, in line with service specification and ensuring patient safety.
  3. Deliver cryotherapy, in line with service specification and ensuring patient safety.
  4. Deliver EMD clinics, ensuring effective management and patient care.
  5. Deliver 6-week baby checks, ensuring timely follow-up and patient engagement.
  6. Deliver joint injections, in line with service specification and ensuring patient safety.
  7. Request imaging tests, in line with IRMER specification and ensuring patient safety.
Leadership and Quality Improvement
  1. Act as the audit lead, effectively utilising the audit cycle and ensuring quality improvement.
  2. Act as the IPC lead, ensuring compliance and undertaking audit as necessary.
  3. Monitor and ensure the safe storage, rotation, and disposal of medicines, adhering to extant legislation.
  4. Participate in local initiatives to enhance service delivery and patient care, ensuring quality improvement.
  5. Support and participate in shared learning within the organisation, promoting knowledge sharing and quality improvement.
  6. Develop an area of specialist interest, taking the lead within the organisation and promoting quality improvement.
  7. Continually review clinical practices, responding to national policies and initiatives where appropriate, ensuring quality improvement.
  8. Participate in the review of significant and near-miss events, applying a structured approach and ensuring quality improvement.
  9. Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required, ensuring quality improvement.
  10. Develop practice administrative and clinical protocols, in line with the needs of the patient and current legislation, ensuring quality improvement.
