Job Title: Nurse Case Manager - Complex Care Specialist

Job Summary:

We are seeking an experienced and skilled Nurse Case Manager to join our team at Compass Associates. As a key member of our organization, you will play a vital role in delivering high-quality care to our clients and ensuring the highest standards of service delivery.

Key Responsibilities:

  • Develop and implement comprehensive care plans for complex clients, taking into account their individual needs and goals.
  • Manage a caseload of clients, providing ongoing support and coordination of care services.
  • Collaborate with multidisciplinary teams to ensure seamless care delivery and effective communication.
  • Conduct regular assessments and reviews to ensure the quality and safety of care services.
  • Develop and maintain strong relationships with clients, families, and caregivers to ensure person-centered care.
  • Provide education and training to non-qualified staff to support them in delivering high-quality care.
  • Oversee and manage staff within the care package, ensuring they have the necessary skills and competencies to deliver safe and effective care.
  • Ensure compliance with regulatory requirements, including CQC regulations and best practice guidance.

Requirements:

  • Registered Nurse Band 6 or above.
  • Full UK Driving Licence and own vehicle.
