Physical Health Nurse Specialist

We are seeking a highly skilled and experienced nurse to join our team as a Physical Health Nurse Specialist. This is an exciting opportunity to work in a dynamic and supportive environment, contributing to the improvement of physical health care for Mental Health service users.
Key Responsibilities- Provide expert advice and support to staff on physical health monitoring and treatment services, ensuring high-quality care for Mental Health service users.
- Act as a resource and advisor to staff, promoting education and training on physical health prevention and promotion.
- Develop and implement health improvement interventions, including smoking cessation and smoke-free clinics, healthy eating, and physical activity initiatives.
- Collaborate with the Physical Health Team to lead on good practice around physical health care and infection prevention initiatives.
- Support the development, completion, and analysis of physical health audits, ensuring evidence-based practice.
- Conduct physical health checks for service users, ensuring the completion of physical health screening and interventions.
- Remain up-to-date with evidence-based practice guidance, ensuring the highest standards of care.
- Support the Physical Health Team in identifying and developing links with health services to facilitate shared care pathways.
- Maintain accurate patient care records, adhering to Trust policy.
- Participate in the care and management of patients at risk of acquiring an infection and/or with an infection, as required.
