We are seeking a highly skilled Clinical Pharmacist to join our team at Woodlands Medical Centre. As a key member of our pharmacy team, you will play a vital role in ensuring the safe and effective prescribing of medication within our Primary Care Network.
About the RoleThe successful candidate will be responsible for conducting clinical medication reviews for patients with single or multiple long-term conditions, where medicines optimisation is required. You will also be involved in reconciling medicines following discharge from hospitals or intermediate care, working closely with patients and community pharmacists to ensure seamless care transitions.
As a Clinical Pharmacist, you will contribute to the provision of out-of-hours/on-call/extended services for the practice and patients. This will include patient-facing and telephone consultations, signposting to other services and/or healthcare professionals where appropriate, while working within the scope of practice and limits of competency.
You will also be responsible for managing repeat prescription requests, resolving queries where possible within the scope of practice, and ensuring patients have appropriate monitoring tests in place when required.
Additionally, you will implement changes to medicines in line with MHRA alerts, product withdrawal or shortage, and other local or national guidance. You will maintain full and complete records of all patient contacts using appropriate clinical templates and coding.
The ideal candidate will have a minimum of 2 years post-qualification experience, with a strong appreciation of primary care prescribing concepts and strategies for improving prescribing. Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience is desirable.
