**About the Role**

We are seeking a dedicated and compassionate individual to join our Care at Home team in Seaboard. As a Support Practitioner, you will have the opportunity to make a real difference in the lives of vulnerable people living in their own homes.

**Key Responsibilities**

  • Provide person-centered care and support to individuals in their own homes
  • Assist with daily living tasks, such as personal care and household chores
  • Encourage independence and promote community involvement
  • Work collaboratively with other healthcare professionals to ensure comprehensive care

**Requirements**

  • Experience working in a care or support role (desirable but not essential)
  • Excellent communication and interpersonal skills
  • Able to work flexibly, including evenings and weekends
  • Ability to pass a Disclosure and Barring Service (DBS) check
