Job Description:
We are seeking an experienced and passionate Care Manager to join our team as a Complex Care Manager. As a key member of our team, you will be responsible for delivering high standards of care to our clients in a community setting.
Key Responsibilities:
- Conduct thorough assessments and implement personalized care plans for clients
- Induct and supervise support workers to ensure high-quality care delivery
- Perform spot checks and risk management to ensure client safety
- Attend meetings with Integrated Care Boards (ICBs) and clients to discuss care plans and progress
- Provide regular reports and updates to stakeholders
Requirements:
- Hold an NVQ Level 3 (or equivalent) in Health and Social Care or a related topic
- Proven track record in managing and coordinating care for clients in a community setting
- Experience in developing and implementing care plans
- Supportive management style with the ability to bring out the best in others
- Advanced communication skills, both verbal and written
- Full UK driving license for client visits
