Job SummaryThis is an exciting opportunity to work in a state-of-the-art PICU & Acute hospital for men, providing high-quality care to individuals with mental health needs.
Key Responsibilities- Closely monitor patient health issues such as obesity, hypertension, diabetes, epilepsy, asthma, COPD, minor injuries, and illnesses.
- Carry out ECGs and Metabolic Syndrome monitoring.
- Coordinate with GP surgery for routine reviews, including cervical smear screening, breast cancer screening, annual chronic health review, etc.
- Liaise with relevant health professionals, including dietician, optician, chiropodist, continence nurse, dentist, Tissue viability nurse, Continence team, Community diabetes specialist team, etc.
- Liaise with pharmacists and contribute to the medicines management committee.
- Support mental health nursing staff in managing minor injuries and wound care management.
- Maintain accurate, detailed reports and records.
- Consult and coordinate with Multidisciplinary team members to assess, plan, implement, and evaluate patient care.
- Promote physical health and well-being to the service users.
- Train staff to perform observation of vitals, perform read ECG, venepuncture.
- Provide health clinic sessions for the service users.
- UK Registered RGN with 2 Years Experience within a Healthcare setting.
- The ability to maintain documentation, undertake audits & demonstrate integrity in your role.
- A good understanding of the latest nursing practices.
- A commitment to patient recovery to effectively monitor, manage & reduce risk.
- Compassion, resilience & dedication to empower & support service user independence.
- Proven experience working alone and also as part of a full MDT team.
