We are seeking a highly skilled and experienced Cluster Frailty Specialist to join our team at Cardiff and Vale University Health Board. As a key member of our multidisciplinary team, you will play a vital role in coordinating the proactive and holistic management of patients with complex needs.
Key Responsibilities- Assess the physical and psychosocial needs of patients with long-term conditions, developing personalized management plans to stabilize their condition and prevent unplanned hospital admissions.
- Work collaboratively with patients, General Practitioners, and community teams to set up management plans and provide education and support to patients and their families.
- Use advanced nursing skills and expert knowledge to identify subtle changes in patients' conditions, analyzing and interpreting complex health problems and associated scenarios.
- Develop and implement advanced care plans to improve communication and facilitate the coordination of patient care, involving the most appropriate professionals from health and social care.
- Work across professional and organizational boundaries, identifying areas for skill and knowledge development and applying these to individual practice to provide continuity and high-quality patient-centered care.
- Registered Nurse with a 1st-level degree or equivalent skills, knowledge, and experience.
- Teaching and assessing qualification or equivalent demonstrable experience.
- Non-medical prescriber or supplementary prescribing qualification or prepared to undertake training within 12 months of commencing the role.
- Good knowledge of anatomy and physiology, as well as up-to-date working knowledge of local and national policy issues pertinent to adults with long-term conditions.
- Experience of working as an autonomous practitioner, case management, and working with adults with long-term conditions.
- Good understanding of working in partnership with statutory and non-statutory agencies to support patient-centered care.
