About the Role

We are seeking a highly motivated and dedicated Community Support Worker to join our team at Trident Housing Association Limited. As a Community Support Worker, you will play a vital role in providing person-centered support to vulnerable adults in the community.

Key Responsibilities
  • Support customers with life skills and empower them to build successful relationships within their community.
  • Develop and implement personalized support plans to help customers achieve their goals and increase their levels of independence.
  • Assess risks and write assessments to draw up risk management plans.
  • Hold regular key worker meetings to review and update customer outcomes.
  • Use Microsoft IT programs to record and track customer progress.
Requirements
  • NVQ Level 2 in Health and Social Care or working towards.
  • Ability to drive and have own transport.
  • Experience of working with customers with mental health needs, including self-harm, personality disorder, autism, schizophrenia, or personal experience.
  • Self-motivated and able to work on own initiative.
  • Experience of professional partnership-working.
