About the Role

The successful candidate will be an exceptional team player with excellent communication and interpersonal skills, committed to delivering high-quality care to our patients. The ideal candidate will have experience in primary care, chronic disease management, diabetes, first contact, and telephone triage.

Key Responsibilities
  • Provide high-quality primary care services to patients
  • Work 36 hours per week
  • Conduct telephone triage and provide first contact and telephone triage services
  • Manage chronic diseases, including diabetes, and provide child immunizations
Requirements
  • Practice nurse qualification with at least two years' experience
  • Experience in long-term conditions, diabetes, or child immunizations is desirable
