About the Role

We are seeking a highly skilled and experienced Children's Community Nurse to join our team at Manchester University NHS Foundation Trust. As a key member of our community nursing team, you will be responsible for providing high-quality care to children and young people with acute and complex needs.

Main Responsibilities
  • Work as part of a multidisciplinary team to provide comprehensive care to children and young people with acute and complex needs.
  • Develop and implement individualized care plans to meet the unique needs of each child and family.
  • Provide education and support to families and other healthcare professionals to promote optimal health outcomes.
  • Collaborate with other healthcare teams to ensure seamless care transitions and coordination.
  • Participate in quality improvement initiatives to enhance the quality of care and services provided.
Requirements
  • Registered Paediatric Nurse with a degree or equivalent qualification.
  • Proven experience in community nursing with a focus on children and young people.
  • Excellent communication and interpersonal skills with the ability to work effectively with families, children, and other healthcare professionals.
  • Ability to work autonomously and as part of a team to achieve high-quality care outcomes.
  • Valid driving license and access to a vehicle.
