Job Summary:

Jupiter Recruitment is seeking a highly skilled and dedicated Band 6 Health Visitor to join our team as a Community Health Specialist. As a key member of our healthcare team, you will be responsible for delivering high-quality health visiting services to children and families in the community.

Key Responsibilities:

  • Health Needs Assessments: Conduct ongoing health needs assessments for all children and families within the community, ensuring timely and effective delivery of services.
  • Service Delivery: Oversee the delivery of universal service key touch points by skill mix team members, ensuring they meet required timescales and KPI targets.
  • Health Visiting Support: Provide health visiting support and advice within the clinic setting, adhering to service models and guidelines.
  • Collaboration: Liaise effectively with colleagues within the organization and partner organizations to ensure timely, well-coordinated, and effective care for children and families.
  • Workload Planning: Plan own workload effectively to meet client needs, clinical activity targets, and KPIs.

Preferred Skills and Experience:

  • Teamwork: Experience of working in multi-disciplinary teams and across organizational boundaries within health and social care.
  • Autonomy: Ability to work autonomously and as part of a team.
  • Public Health Role: Knowledge of the public health role of contemporary health visiting.
  • Legislation and Directives: Current knowledge and understanding of reports and legislation affecting community nursing, including current PHE directives.
  • Computer Literacy: Proficiency in computer software and systems.
