Position Overview

Primary Care Careers is seeking a dedicated and proactive Chronic Illness Care Nurse to become a vital part of our collaborative and supportive healthcare team. This role offers flexibility with full or part-time opportunities, encompassing various weekday shifts from 8:30 AM to 6:30 PM.

This multifaceted position emphasizes the management of chronic illnesses while also allowing the nurse to leverage their expertise in areas such as wound care, health education, women's health, and medication oversight.

Key Responsibilities

  • Conduct comprehensive evaluations of patients with chronic conditions such as diabetes, hypertension, and cardiovascular diseases, developing and assessing tailored treatment plans to enhance overall health.
  • Review and adjust lifestyle choices and medication regimens as necessary.
  • Diagnose and manage both acute and chronic health issues as required.
  • Reassess medication needs following any acute health events or hospital admissions.
  • Lead chronic disease management clinics, providing direct patient care and coordinating referrals to specialized services when needed.
  • Identify and support patients at risk of developing long-term health conditions, implementing preventive strategies to safeguard their well-being.
  • Prescribe medications in accordance with established guidelines and within the scope of practice.
  • Assist physicians with minor surgical procedures as needed.
  • Implement vaccination initiatives for both adults and children.
  • Guide patients in the effective use of prescribed and over-the-counter medications.
