Job Title: Field Care Supervisor

We are seeking a skilled and compassionate Field Care Supervisor to join our team at Nobleliveincare. As a Field Care Supervisor, you will play a vital role in ensuring the delivery of high-quality care support services to our service users.

Key Responsibilities:
  • Support and supervise care workers to ensure they meet the highest standards of care.
  • Conduct assessments and reviews of individual services to ensure our service users receive the best possible care.
  • Collaborate with care coordinators to plan effective care worker rotas that align with the specific needs of our service users.
  • Monitor care worker performance and provide feedback and guidance as needed.
  • Participate in multi-disciplinary reviews of service users' needs to provide comprehensive and coordinated care.
Requirements:
  • Proven experience in conducting initial risk and needs assessments for individual service users.
  • Ability to work in partnership with service users, their families, carers, advocates, and healthcare professionals to ensure seamless delivery of care packages.
