Job Title: Band 5 Community Nurse

We are seeking a highly skilled and dedicated Band 5 Community Nurse to join our team at Mid Cheshire Hospitals NHS Foundation Trust. As a Community Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in their own homes and communities.

Key Responsibilities:
  • Assess and manage the health needs of patients, developing and implementing personalized care plans to meet their physical, social, and emotional needs.
  • Work collaboratively with other healthcare professionals and agencies to ensure seamless care delivery and optimal patient outcomes.
  • Provide skilled nursing care, including wound care, leg ulcer assessment and management, and administration of medication via syringe pump.
  • Conduct home visits to assess patients' needs and provide education and support to patients and carers.
  • Participate in the development and implementation of care pathways and protocols to improve patient outcomes and reduce healthcare costs.
  • Contribute to the development and maintenance of clinical skills and knowledge through ongoing education and training.
Requirements:
  • Registered General Nurse/Registration Imminent
  • UK driving licence and use of car
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Flexibility to work across the footprint of Central Cheshire Integrated Care Partnership
