We are seeking a dedicated Clinical Nurse Leader for Adult Care to become part of a reputable and innovative organization that specializes in delivering complex care solutions within the community, focusing on adult care needs.

The ideal Clinical Nurse Leader will be tasked with providing direct support to adult clients and their families, while also developing customized care plans tailored to individual requirements.

You will collaborate closely with a dedicated team of caregivers, offering training and guidance to enhance their clinical competencies and service delivery.


Essential Qualifications for the Clinical Nurse Leader:
- Registered General Nurse (RGN) with a valid NMC PIN
- A nurturing management approach that fosters team development
- Recent experience in tracheostomy care and ventilator management within the last two years
- Excellent verbal and written communication skills for effective interaction with patients, families, and the multidisciplinary team (MDT)
- Suitable for Band 5 / Band 6 Nurses with proven leadership and management experience with Healthcare Assistants (HCAs)
- A full UK Driving Licence is preferred but not mandatory

Key Responsibilities for the Clinical Nurse Leader:
- Create and implement individualized care plans based on client-specific needs
- Effectively lead the MDT through strong communication and collaborative teamwork
- Provide continuous monitoring, support, and assessment of clients' physical and psychological needs within your caseload
- Administer nursing interventions, including the management of medication delivery
- Establish and nurture meaningful relationships with clients' families
- Collaborate with community teams, Continuing Healthcare (CHC) nurses, and Clinical Commissioning Group (CCG) commissioners to develop and maintain tailored care packages
