Care Specialist

We are seeking a highly skilled and compassionate Community Specialist Palliative Care CNS to join our team at University Hospitals Dorset NHS Foundation Trust.
The successful candidate will provide expert palliative care symptom control advice and psychological support to patients with advanced illness and complex needs, as well as their families and carers.
Key responsibilities include:
- Providing nursing expertise and palliative care advice in symptom control and psychological support for patients with advanced, progressing life-limiting illness with complex needs.
- Acting as an expert resource for patients, families/carers and health and social care professionals caring for them.
- Supporting Advance Care Planning and other sensitive conversations utilising good communication skills.
- Promoting collaborative working and personalised care and support 'what matters most' to patients and their families.
- Managing a clinical caseload of patients under supervision and proactively supporting and co-ordinating the individualised care and support needs for patients and their families acting as patient advocate, through collaboration and participation with the rest of the multi-disciplinary team within primary and secondary care.

We are looking for a candidate with:
- Palliative Care experience
- Experience of multi-professional working
- Ability to manage own workload effectively and independently
- Post registration qualification in palliative care or relevant subject or working towards
- Experience of working in the community
- History taking and physical assessment qualification
- Evidence of caseload management
