About the Role:
We are seeking a highly skilled and experienced Senior Physiotherapist to join our Reablement Therapy team at Somerset NHS Foundation Trust. As a Senior Physiotherapist, you will play a key role in delivering high-quality patient-centered care and specialist rehabilitation to adults referred to our Intermediate Care Bedded Pathway Service.
Main Responsibilities:
- Deliver high-quality patient-centered care and specialist rehabilitation to adults with complex needs
- Work collaboratively with multidisciplinary teams to develop and implement individualized care plans
- Conduct thorough assessments and develop effective rehabilitation programs to promote independence and improve functional ability
- Provide education and support to patients, families, and caregivers to promote self-management and empowerment
- Contribute to the development and implementation of evidence-based practice guidelines and protocols
Requirements:
- Diploma/degree in Physiotherapy and HCPC registration
- Postgraduate experience relevant to the post, with a strong focus on rehabilitation and community-based care
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams
- Good time management and organizational skills, with the ability to prioritize tasks and manage competing demands
- Leadership skills, with the ability to mentor and support junior staff members
