We are seeking a skilled and compassionate Care Coordinator to join our Psychiatric Intensive Care Team at Pennine Care NHS Foundation Trust. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality care to our service users.
Key Responsibilities- Plan, organize, and deliver ward-based activity programs for individual and group patients within the Psychiatric Intensive Care unit.
- Provide specialist support to patients and work collaboratively with the multidisciplinary team.
- Adopt a non-judgmental approach with patients, based on respect and dignity.
- Undertake ward-based activities with patients that promote recovery, are valued, and aim to increase independence.
- Contribute to the development of an overall ward-based activity program for the service that promotes involvement and choice, and provides a range of therapeutic, recovery-focused, and age-appropriate activities on the unit.
- Design, manage, and promote health improvement programs for patients.
- Support medical and nursing staff in the completion of physical health monitoring.
Our team provides short-term intensive assessment, treatment, and support to service users who may be experiencing a mental health crisis and often present with complex mental health needs.
To maximize service user recovery, our teams work in a trauma-informed way and utilize the RAID model of care. Our aim is to work in the least restrictive way possible while maintaining patient safety.
Upon joining our team, we will ensure that successful candidates undergo both mandatory and bespoke training to fully equip them with the skills required to work within our PICU environment.
Person SpecificationWe are looking for an individual with:
- NVQ Level 3 Health and Social Care or equivalent short courses and/or experience.
- PMVA Qualification or commitment to complete (Trust-funded).
Desirable criteria include evidence of further education or qualifications, such as art, music, or reminiscence, and experience of working in mental health or dementia services.
