About the Role:

We are seeking a highly skilled and experienced Occupational Therapist to join our Community Independence Service (CIS) team in the Royal Borough of Kensington and Chelsea. As an Occupational Therapist, you will play a vital role in supporting adults with complex health and social care needs to maintain their independence and wellbeing.

Key Responsibilities:

  • Conduct comprehensive assessments of individuals' capabilities and develop personalized reablement plans to support their independence.
  • Work collaboratively with a multidisciplinary team, including social workers and independent living assessors, to provide holistic support to service users.
  • Guide and mentor colleagues to promote a culture of learning and continuous improvement.
  • Develop and maintain strong relationships with service users, their families, and carers to ensure person-centered care.
  • Stay up-to-date with best practices and evidence-based interventions to inform your work.

Requirements:

  • Registered Occupational Therapist with a community-based background and significant experience in carrying out OT assessments.
  • Strong communication and interpersonal skills to work effectively with diverse individuals and teams.
  • Ability to work in a fast-paced environment and prioritize tasks to meet deadlines.
  • Proficiency in electronic case management systems and care act 2014 assessments.
  • Comfortable with moving and handling work and ability to work in a physically demanding environment.
