We are seeking a highly skilled Nursing Associate to join our Intensive Home Support Service, providing community care for patients in their own homes.
The successful candidate will work with a diverse population, utilizing excellent communication and interpersonal skills to deliver holistic care within a supportive team environment.
Key responsibilities include:
- Providing direct patient care to patients in the community
- Working collaboratively with a multidisciplinary team, including Advanced Clinical Practitioners, Therapy Teams, and Mental Health Practitioners
- Contributing to shifts covering 08:30-22:00 hours, 7 days a week, 365 days a year
The post holder will be responsible for:
- Communicating sensitive information effectively and improving communication using a range of strategies
- Handling information and data in line with national and local policies and legislation
- Applying and promoting safe and effective practice that places the individual and/or family/carer at the centre of care
- Maintaining active status on the NMC register and adhering to the NMC code of conduct
The ideal candidate will possess:
- A Nursing Associate: Level 5 Foundation Degree
- Associate Nurse on the NMC register
- GCSE Level 2 English Language or equivalent
- GCSE Level 2 Maths or equivalent
