Job Summary

We are seeking a highly skilled and experienced Nurse Case Manager to join our team at Leaders In Care Recruitment Ltd. As a Community Nurse Case Manager, you will play a vital role in supporting clients with complex care needs in the North West region.

Key Responsibilities
  • Conduct comprehensive assessments in the community to identify client needs and develop personalized care plans.
  • Train and assess the competencies of support workers to ensure they are equipped to provide high-quality care.
  • Collaborate with a multidisciplinary team to ensure seamless care delivery and effective communication.
  • Attend meetings and make referrals to ensure clients receive the necessary support and services.
  • Hold team meetings to discuss client progress, address concerns, and provide guidance and support.
  • Manage a team of non-clinical case managers to ensure efficient and effective care delivery.
Requirements
  • Registered General Nurse (RGN) or Registered Sick Children's Nurse (RSCN) qualification with a strong background in complex care.
  • Experience in tracheostomy and complex care management, including peg feeds and ventilator management.
  • Valid car driver's license and flexibility to travel around the North West region.
  • Experience caring for both adults and children with complex care needs.
Benefits
  • Competitive annual salary of £43,000.
  • £3,000 car allowance.
  • Mileage expenses.
  • Full-time hours: 37.5 hours per week, Monday to Friday.
