We are seeking a skilled and compassionate Palliative Care Support Worker to join our team at Tameside and Glossop Integrated Care NHS Foundation Trust. As a key member of our team, you will play a vital role in delivering high-quality care to patients with palliative diagnoses.
Key Responsibilities- Delivering Patient Care
- Provide nursing and social care to patients in their own homes, under the direction of the Palliative Care Respite Team Coordinator and supervision of the District Nurse.
- Assist with personal care, including pressure relief, personal hygiene, continence care, and oral hygiene.
- Supporting Carers
- Offer flexible sitting and support services to enable carers to have respite periods.
- Prepare drinks and light meals for patients as required.
- Documentation and Communication
- Maintain accurate and up-to-date documentation.
- Communicate effectively with patients, carers, and healthcare professionals.
- Teamwork and Development
- Attend regular team meetings and contribute to team development.
- Participate in clerical work, such as stock ordering and filing, as delegated by the Team Coordinator.
- Essential Criteria
- Good interpersonal skills and effective communication.
- Ability to work in a team and support colleagues.
- Ability to time manage and use own initiative.
- Computer literacy and ability to use software applications.
- Desirable Criteria
- Experience working in a healthcare or social care setting.
- NVQ Level 2 or 3 in care or equivalent level of skill and knowledge.
