As a Medication Management Specialist, you will play a pivotal role in supporting clinical pharmacists, community pharmacists, and other members of the multidisciplinary team within the Primary Care Network (PCN). Unlike clinical pharmacists, your responsibilities will not include prescribing or making clinical decisions; instead, you will operate under supervision to ensure the optimal and efficient utilization of medications.

Key Responsibilities

The primary objective of this role is to spearhead enhancements that maximize safe and cost-effective best practices in medication prescribing, ultimately improving patient care quality. Your contributions will assist patients in deriving maximum benefit from their medications by:

  • Transitioning medications in accordance with established protocols.
  • Streamlining repeat prescribing processes, including the promotion of repeat dispensing and online ordering.
  • Minimizing clinical risks and striving to reduce medication waste.

Additionally, you will be tasked with fostering a deeper understanding of medication optimization principles among practice teams and advocating for best practices aligned with therapeutic advancements. This will involve aiding the PCN in meeting national standards, implementing NICE guidelines, and utilizing medication optimization initiatives.

Responsibilities Overview

Clinical Duties

- Engage in patient-facing roles to ensure effective medication use through collaborative decision-making.

- Execute medication optimization tasks, including effective administration checks and supporting medication reviews.

- Assist in medication reviews and reconciliation for new care home patients, ensuring synchronization of medications during patient transfers.

- Collaborate with the Clinical Pharmacist in Structured Medication Reviews (SMR) and organize necessary monitoring tests.

- Provide expertise to address public health and social care needs, including lifestyle advice and service information.

- Manage shared care protocols and liaise with Clinical Pharmacists for complex cases.

- Support initiatives aimed at antimicrobial stewardship to mitigate inappropriate antibiotic prescribing.

Technical and Administrative Duties

- Assist the PCN multidisciplinary team in ensuring efficient medication optimization processes.

- Implement effective ordering and return processes to minimize medication wastage.

- Provide training on the legal and safe handling of medications, including the Electronic Prescription Service (EPS).

- Promote Electronic Repeat Dispensing (eRD) and online ordering.

- Cultivate relationships with pharmacy professionals and multidisciplinary team members to enhance integration across health and social care.

- Support practice reception teams in managing general prescription requests to allow GPs and clinical pharmacists to focus on more complex cases.

- Aid in the implementation of national prescribing policies and guidance within various healthcare settings.

- Contribute to the PCN's objectives related to quality improvement and enhanced services.

Qualifications and Experience

Essential Qualifications

  • Professional registration with the General Pharmaceutical Council (GPhC).
  • Completion of specific qualification and training requirements as outlined by the GPhC.
  • BTEC/NVQ level 3 or equivalent in pharmaceutical sciences.
  • Evidence of ongoing professional development (CPD).

Essential Experience

  • Proficient in using GP clinical systems and Microsoft Office applications.
  • Ability to analyze and interpret prescribing data.
  • Strong attention to detail and accuracy in work.
  • Organized approach with the ability to prioritize workload effectively.
