We are seeking an experienced Social Worker to join our Long Term Team in Newcastle. As a Social Worker, you will play a vital role in providing long-term support and guidance to individuals, helping them to achieve their goals and improve their overall well-being.
Key Responsibilities
• Provide long-term support and guidance to individuals, helping them to achieve their goals and improve their overall well-being
• Conduct thorough assessments and develop personalized support plans to meet the unique needs of each individual
• Build strong relationships with individuals, their families, and other stakeholders to ensure effective communication and collaboration
• Work closely with other professionals, such as healthcare providers and educators, to ensure comprehensive support
Requirements
• 2+ years of experience as a Social Worker, preferably in a long-term care setting
• Strong knowledge of social care principles and practices, including assessment, planning, and intervention
• Excellent communication and interpersonal skills, with the ability to build strong relationships with individuals and stakeholders
• Ability to work independently and as part of a team, with a strong commitment to delivering high-quality support
